歸功于CT在耳科的應用，Minor等于1998年報道了以聲音和壓力誘導的眩暈為主要癥狀的上半規管裂綜合癥，并且通過顱中窩入路進行上半規管填塞獲得了治療效果(Minor et al., 1998)。經過15年發展歷程，目前對該病的診斷已經成熟，顳骨高分辨CT仍然是診斷上半規管裂的金標準，用3D fast imaging employing a steady-state acquisition序列進行的MRI具有100%的靈敏度和陰性排除率(Browaeys et al., 2013)。治療方法也在不斷更新，Mikulec等人嘗試經顱中窩入路用顱骨裂層行上半規管頂壁重建，但是無效(Mikulec et al., 2005)。Crovetto等人于2007年報道經乳突入路行上半規管頂壁重建并獲得良好效果(Crovetto et al., 2008)。但是，該方法對乳突及內耳的騷擾還是較大。Nikkar-Esfahani等人于2013年報道圓窗封閉這一微侵襲外科治療上半規管裂的方法，獲得良好效果(Nikkar-Esfahani et al., 2013)。具體方法為，經外耳道徑路進入鼓室，辨認圓窗龕并清除其表面的粘膜，辨認圓窗膜并分別用骨蠟、肌肉和筋膜進行三層封閉。2014年，Silverstein等人報道了圓窗加固手術治療上半規管裂的多中心研究結果，進一步肯定了這一微侵襲外科手術的治療效果，其中Michigan耳研所的 Kartush在加固圓窗的同時，將筋膜放置于鐙骨底版周圍，獲得了很好效果，并認為這是一種更加安全和有效的方法(Silverstein et al., 2014)。我們于2014年在國內首先用改良的圓窗和卵圓窗加固術治療外傷性上半規管裂的患者（圖1），獲得很好的效果(Zou et al., 2014; 鄒靜 et al., 2015)。該手術在局部麻醉下邊可以完成，顯著降低了手術的風險，因為不進入內耳，對內耳無任何騷擾。
